NOTE 4: LETTER: From, The Commonwealth of Massachusetts Soldier's Home, Chelsea, April 8, 1938; To Whom It May Concern: Mr. John A. Hackett was admitted to the Soldier's Home hospital February 4, 1938 and remained in hospital under treatment for a serious heart condition until March 28, 1938 at which time he was discharged to his home. Patient has been advised to retire from his occupation as fireman because his condition requires mental and physical rest which he should adhere to during his lifetime. (signed) L.P.Fincle, M.D.

 

NOTE 5: LETTER: From City of Lynn, in City Council August 23, 1938, Ordered that WHEREAS, John A. Hackett, 100 Den Quarry Road, Lynn, Massachusetts, a permanent member of the Lynn Fire Department, having been a call member of said department for six (6) years and a permanent member thereof for eighteen (18) years, who has reached the age of 49 years, and is now totally physically incapacitated for further duty as a member of the Lynn Fire Department, as per certificate of Walter L.Hearn, M.D., 45 Franklin Street, Lynn, Massachusetts, hereto annexed and made part hereof, has requested retirement and pension in accordance with the provisions of the Revised Ordinances of the City of Lynn, and, WHEREAS, said retirement and pension has been recommended, in writing, by William F. Welch, Chief of the Lynn Fire Department, and WHEREAS, the said John A. Hackett, in the opinion of the City Council of the City of Lynn, is entitled to said retirement and pension, it is therefore ORDERED, that the City Council of the City of Lynn hereby retires from active service as a member of the Lynn Fire Department, the said John A. Hackett and awards him an annual pension of $1050., being one-half the amount of compensation received by him at the time of his retirement, payable monthly, at the rate of $87.50, and the expense incurred hereunder, until further order of the City Council, shall be charged to the account of "Fire Department". This order shall take effect upon its adoption and approval

 

NOTE 6: CERTIFICATE OF MEMBERSHIP: Upon satisfactory proof furnished to the Secretary of the Massachusetts Permanent Firemen's Benefit Association of the death of JOHN A. HACKETT, a member in good standing, the Association will pay to the herein named beneficiary, the sum of One Thousand Dollars ($1,000) to be paid to his SISTER, MABLE F. DOBLE. Witness my hand this EIGHTH day of DECEMBER A.D. 1931. JOHN A. HACKETT, in the presence of GUY R. STORER. I, JOHN A. HACKETT do hereby revoke my former directions as to the payment of said benefits and now direct such payments be made to his WIFE HARRIETTE B. HACKETT, Witness my hand this 21st day of AUGUST A.D,. 1933 JOHN A. HACKETT in the presence of M.E. BALLARD.
